Depends on the hospital. Some just forward the bill to an independent collection agency, and they then proceed to harass you via mail/phone at all hours of the day until you pay up + interest. Others take you directly to court and have the state garnish your wages. I'd work with the hospital and see if you can't set up a payment schedule. That, or become homeless in which case they won't bother and will probably treat you for free 4eva (well, at least in instances when you're on the verge of death... maybe).

meanwhile i had surgery on my broken foot, had shitty insurance, and ended up paying somewhere around $10k when all is said and done. not counting the ~$100 a month i paid for insurance in the 7 years prior when i never went to a doctor or hospital

i once called hospital and said i didnt have insurance and they lopped my bill way down too lol
