The hospital sends you a bill for a hundred grand, insurance pays for it, that payment goes through the hospital right back to the company. now they not only have justification to dial their premiums up into the sky, but also squeeze vastly more money out of the public treasury via the subsidies for poors, maybe even prevent poors and nonwhites from even being able to infect the pristine white halls of healing altogether by cranking up copays on that imaginary inflated bill, and solidify their regional monopolies.

Under the new system this is preferred because it lets them ignore the concept of medical loss ratio by just adjusting their own internal pricing schemes to justify charging whatever they want so that the medical loss ratio aligns with their desired profit goals. since insurance will be subsidized by the government, they have a buffer against public outcry at the prices. and of course if they have an existing monopoly of the local insurance market they can still force other hospitals to swallow the negotiated rates.

Sooo I'm not sure what you mean by strongarm. I'm guessing that the insurance company says that they'll only cover up to a certain amount, which isn't close to the amount of the actual cost of the procedure or whatever. And then the hospital charges the reasonable amount but never gets paid the full amount because nobody can afford the difference from the insurance companies coverage amount?
So a patient with BCBS walks in and has an mri and either pays the 300 and gets paid back, or pays nothing and the hospital bills BCBS the 300, or the patient gets a bill for like 350 which BCBS has negotiated with them for people on a certain plan. BCBS pays 300 and the patient eats the other 50. Depends on the victims actual insurance package. Or, they can get told the insurance company actually won't pay it because of some contractual technicality but they're still in BCBS so they get a discount rate and only pay like 300 or 500 or whatever. This is why private hospitals tend to have more billing clerks than actual doctors.
However a patient without insurance walks in and the hospital charges like 1500 dollars because not only are they not covered by the agreement with BCBS but the hospital has to try and get extra money from someplace to pay for the machine.
The federal government does the same thing with medicare for olds and medicaid for poors but a lot of independent doctors and smaller clinics have found the easy way to work around that is to just not accept the poors as patients because medicaid doesnt pay them shit compared to what they can bill insurance or paying patients.

a more accurate scenario: The hospital charges you $2500 for an MRI scan. They also charge your insurance company $2500 for the very same scan. The insurance company pays the $2500, funded with your own money, after a lengthy and protracted negotiation/paperwork battle where you eventually convince them that yes, this MRI scan is covered by your insurance and youre not going to just allow them to not pay it. The hospital continues charging you $2500 for the already paid for scan, and eventually turns you over to a 3rd party collections agency. In the meantime you have lost your job for taking a single sick day off, and will now receive single-payer health care from the confines of a debtor's prison
